Cost per sqft for ~4000sqft of concrete pad and apron

I am looking for a rough price for the following:

240 lnft of 6' wide sidewalk

600 lnft of 3' wide apron (doesn't need to be finished as well as sidewalk)

3 concrete pads about 200 sqft each, 6" depth.

Work is to be done in Toronto (Etobicoke).

This is for a commercial property.

Price should include concrete, forming, substrate, etc.
