When a Contractor comes gives quote and lists the job removing old fence cement and all but cuts posts off ground level and leaves it? Is this proper?
What is a fair price per foot for everything, for a fence 4x6, posts bastion 6 foot fence all in debri removal, site cleaned up?

Typically cutting posts just below ground level is acceptable assuming the customer is ok with the post positions being off-set slightly. Expect to pay upwards of $150/post to dig and dispose of old concreted posts, so if you're ok with just cutting them off at ground level you'll save lots of time and money.
Expect to pay around $35 Lin/ft and $100/post. Ball park range.

It is just common sense to prepare an clean up the site properly before and after the work, normally this included in the quote subsecuently the price.

Yes it is standard practice to leave the concrete in the ground. I've only had one client 35 years want the old concrete removed which I did but charge them as much as the fence cost and it wrecked a bit of their backyard which I told them it would do.
Well that all depends I always try and remove the old post with the concrete and made a custom jack to do so, but if the post is rotted into the concrete I make sure the customer is ok with me cutting it off and putting new post in beside the old one. But I always tell my customers when I give them a quote. And I usually charge 40 to 50 a lnft to remove and dispose old material and that includes all new materials and labour
