We're not the handiest of households, so I have no idea how to even go about repairing this. All I can say is that it looks like the threshold has actually sagged somehow, possibly due to some settling the house has gone through.
Any advice on what to do about this would be great.

I don't want to be the bearer of bad news, but being an exterior door there are two problems:
1. Exterior doors are a solid frame so removing just the base is typically not possible. It may be if it an older door, having not seen it I can't say for sure.
2. If the threshold is sagging that doesn't bode well. It could likely mean there is rotting under the threshold due to water intrusion.
In either event, you're going to want to contact a contractor or a window and door business to take a look at it and give you a more definitive answer.

From the sounds of it you could shim it underneath the threshold. My main concern is why it's like that in the first place either it's from heavy traffic or the subfloor underneath is water damaged. Please call a trusted Pro in your local area
