Does the contractor determine the cost by the decking floor space or the structure overall?

Both are factors in costing a deck. Other major variables may include the height of the deck off the ground, whether or not it is a covered deck, the materials chosen, the amount and complexity of stairs and rails, skirting, lighting, wether or not you would like to incorporate utilities - Electrical, Water, Gas, etc for outdoor kitchens or more!

Depending on the size and complexity overall, the substructure required by code may be one of the most costly factors.

I think that is irrelevant, unless you are coming to be contractor and how does one calculates and conducts his business is just his or hers to know.
