Does depend on a few things but generally wood burning is 16" and gas can be 12", or less in some cases.

You need to have non combustible material in front of a fireplace for the first 18 ." there are no specific requirements for the size of a hearth.

Not aware of any regulations re. distance from firebox but I would go for approx. 12'' based on personal experience.

Ontario building code 9.22.5.1. Hearth Extension... Hearth must extend at least 400mm our from fireplace opening (just under 16"). Older hearths tend to be shorter due to old regs.
