Need to have my basement concrete cut open and Bathroom plumbing, sewer lines, drains, Hot & cold supply lines roughed in/installed. Who does this a Plumber or a GC?
Typically a plumber would handle that from start to finish. However in some cases a general contractor may provide the concrete cutting/patching portions of the job.

It depends on your arrangement with GC it a plumbing company.
Both are capable of doing it.
If GC had his own plumbers, he will coordinate with them.
If you are outsourcing plumbing company separately (which is not a good idea in ny opinion) they will coordinate with GC
A licenced plumber with a permit.
Concrete cutting varies plumber to plumber. Depending on the plumbing company size or experience. Some plumbers outsource it, some handle it themselves or have homeowner hire concrete guys. The GC we do business with, takes care of all that for us, so we just do our piping rough in. Concrete floor cutting is not always necessary, there's a way to add a basement bathroom without adding extra costs.
