Concrete deck

How much would a concrete deck (patio) cost per square foot? I would be about 4 feet above ground and about 15' x 38'.

For something like that your looking at about 20k plus depending on design and finish of concrete. You would need drawing for this also which add on to the cost.

You are looking at $4-$6 per sq ft.

That would cost you about 6-8$ a square foot.
