Determination of errors made that led to a failed self-level concrete installation

I would like to reach out to some more concrete and floor preparation trade experts and ask for an independent and unbiased assessment of what could have gone wrong during a self-level concrete pour. Please see photographs for review.

The concrete is supposed to be walk-able after 4 hours, but the after 5 days, the following issues are found:

- Concrete has shrunk in many areas and floor is no longer level

- Sections of the floor are still wet with a sludge-like texture

- Lots of surface liquid pooling with a yellow tint to the color.

- Color of the pour is multi-color instead of a uniform gray.

- Soft texture and loss of density and firmness.

- Significant loose sand on the top surface that can be brushed away (see pile)

- Water circles where buckets of mixed SLC were poured on the floor

- Concrete is not smooth; has curled or peeled on the upper surface.

- Very wet for the first 3 days. Could not even walk on it which is unusual for SLC.

Please see photos. Any overall assessments, ratings, or comments would be greatly appreciated so I can better understand why this was the outcome.

If you have a lot of verifiable industry experience doing self-level applications and can come on-site in Bradford to perform an in-person closer inspection to determine what errors led to this outcome, I would also be very interested in hearing from you as well.

You have excess moisture coming up through the concrete, if SLC mixed right, otherwise it lies with the product & mix or a combination of both
