Based on the scope of the work you are describing, yes you would need a permit.
You can have your contractor obtain one on your behalf but you will need to arrange to have him as an agent on the paperwork when applying for the permit.

Yes as others have posted, you will need a permit.
Generally I obtain the permits on my clients behalf. But if you have your own drawing etc, you can go to the building department yourself and get one. Depending on the municipality you are in there is a length of time the permit is good for. So don't apply for something you are planning many months down the road.

Yes my dear you do need a permit. Get an architect to drawing for you and she will also apply for permit on your behalf. This will require a site visit. You will get architect drawing with structural reinforcement info.
I am doing a job for my for a client in April in mississauga. My architect got the permits ready and we are just waiting for the frost to clear up to start the project.
