What can I expect to pay my General Contractor? Should I expect to be charged for time + materials? A flat fee? This is over and above any out of pocket expenses.

Cost of materials + 30-45% is typical
or
Cost of materials + 10% markup and a Labor Rate which can be anywhere between 85-150/hr depending on the work.
