How do I report a technician?

How do I report a licensed technician who is signing off on jobs without actually doing the checks/balances? I have a form completed by a tech (with his cert #) stating he completed the inspection - before the unit was actually installed completely. We've been having issues for over a year and now know for a fact there was no way he could have balanced the unit or certified it at the time he says he did.
